A Personal Profile Form

For people who don’t want to prepay or prearrange their
funeral service, a personal profile form is a very useful tool.
The questions on a personal profile form allow you to list
all the information required to register a death in the State
of Victoria.

The form also has places to record basic funeral requests if
you wish to complete these. Other information that can be
recorded on the form include the whereabouts of your will
and your bank and pension details. These last details are not
required by your funeral director but may be useful
information for your family and / or your executors.
